Impact of N-Acetyl cysteine in Reducing Urinary B2 microglobulin as Bio-marker for Renal injury Post ESWL in pediatrics

Abstract

Purpose B2 microglobulin levels in urine have been measured to determine the role of N-acetylcysteine in renal protection against the oxidative, vascular and ischemic effects of extracorporeal shock wave lithotripsy. Patients and methods It is planned to conduct a prospective randomised trial on 56 paediatric patients under the age of 18 years with a renal pelvic single stone at ESWL. Patients with radiolucent stones , kidney stone larger than 20 mm in diameter, chronic kidney diseases (CKD) , urological congenital anomalies. Patients who had coagulopathy problems were excluded from the study. Two groups of patients were randomly assigned. N-Acetyl cysteine have been given to Group A for 3 days before and 3 days after ESWL (100 mg three times daily for pediatrics till seven years and 200 mg three times daily from seven to eighteen year). Group B have been given placebo. The levels of B2 microglobulin in both groups were measured 3 days before and after ESWL. Results there was no significant difference founded in urinary B2 microglobulin at pre Eswl in both groups but at post Eswl Group (A) was significantly lower than group (B). Stone-free rates at 2 weeks did not differ statistically from one group to another. Conclusion As far as pediatrics is concerned, N-acetylcysteine may show promising results in preventing ESWL-induced renal injury.
