Comparison between Superomedial Pedicle and Inferior Pedicle Breast Reduction in Macromastia.

Abstract

Background: Over the past century, reduction mammaplasty has undergone a variety of inventive and creative developments. These variants are based on the design of the skin pattern and the choice of pedicle for nipple areolar complex transposition. This study aimed to evaluate and assess the efficacy and outcomes, both functional and cosmetic, of the superior-medial and inferior pedicles in reduction mammoplasty and to assess if supero-medial based pedicle (SMP) mammoplasty has superiority over the inferior one regarding cosmetic outcome, operative time and patient satisfaction. Methods: We conducted a prospective randomized comparative study with 6 months follow-up, included 24 patients who underwent reduction mammoplasty were randomized into 2 groups: Group A (odd numbers) who used inferior pedicle technique. Group B (even numbers) patient who used superior pedicle technique. Superomedial breast reduction or inferior pedicle were carried out in a standard fashion with Wise skin pattern. Results: we found that the mean operative time was significantly shorter in the superomedial pedicle group (176.67±12.31 minutes) compared to the inferior pedicle group (199.17±16.21 minutes), in addition, regarding the rate of complication in our study was 29.1% for all reductions. As to the cosmetic outcomes, our study showed that the inferior pedicle group showed a significantly higher rate of bottoming-out (33% vs. 8.3%) in the superomedial group. Regarding patients’ satisfaction, superomedial pedicle breast reduction was superior to the inferior pedicle. Conclusion: that Egyptian patients can benefit from the safety, dependability, and suitability of both superior and inferior pedicle procedures for breast reduction
