Evaluation of Acetabular Remodeling after Open Reduction ‎in Developmental Dysplasia of the Hip

Abstract

Aim of the work: The aim of this study was to evaluate clinical and radiological outcome of ‎acetabular remodeling following open reduction in cases with developmental dysplasia of the hip ‎‎(DDH) and to assess ‎

Methods: This clinical study included 40 patients (48 hips) with DDH who underwent clinical and ‎radiological evaluation for detection of acetabular remodeling after open reduction .‎

‎ Results: According to clinical evaluation using McKay criteria the majority of cases were good in 32 ‎cases (66.6%). According to radiologic evaluation the acetabular index (AI) dramatically diminished ‎from 39.10±3.86 preoperative to 25.40±2.19 postoperative. CEA rose markedly from 23.65±3.05 at ‎post to 26.62±3.93 at the end of follow-up. According to Severin criteria majority of cases were type ‎II in 30 cases (62.5%). Only 4 cases (8.20%) experienced complications. ‎

Conclusion: The physiological acetabular remodeling process suggests that allowing the hip to develop ‎naturally after open reduction in DDH, especially for children operated under three years , as this is the ‎optimal period for acetabular growth (the first four years of life). When hip fail to develop so pelvic ‎osteotomy must added. ‎
